Non-NHS Services and Statement of Fees
Whitley Road Medical Centre provides NHS general medical services to registered patients. However, some services requested by patients, employers, insurers, solicitors or other organisations are not included within the NHS contract.
Where a service is not funded by the NHS, the practice may charge a private fee to cover the clinician’s time, administrative work and any associated costs.
Examples of non-NHS services
These may include:

Private medical reports and insurance forms

Letters requested by patients for schools, employers, housing providers, travel companies or other organisations

Private sick notes or certificates not covered by the NHS

Fitness-to-travel, fitness-to-exercise or fitness-to-participate letters

Holiday cancellation forms

Private medical examinations

Taxi, HGV, PSV, sports, fostering or adoption medicals

Requests for copies or completion of forms that fall outside NHS requirements

Private prescriptions or vaccinations where these are not available through the NHS

Occupational health information and reports

Solicitor or legal reports

Access to medical records where a charge is permitted by law
This list is not exhaustive. Reception will advise you whether your request is covered by the NHS and whether a fee applies.

Fees may vary depending on the complexity of the request, the amount of medical information that must be reviewed and the time required to complete the work. We will confirm the fee before the work is undertaken.
Payment
Payment is normally required before the document, report, examination or certificate is completed.
Payment can be made by:

Please retain your receipt.
How long will my request take?
Non-NHS work is completed outside the practice’s core NHS clinical responsibilities.
Complex reports, legal requests and forms requiring a detailed review of the medical record may take longer. Reception will provide an estimated completion timeframe when your request is accepted.
Please do not book an urgent clinical appointment solely to request completion of a form or letter.
Important information
A clinician can only provide information or an opinion that is supported by the patient’s medical record and their professional assessment.
Completion of a form or letter is not guaranteed. A clinician may decline a request where:

There is insufficient information available

The request falls outside their professional competence

An examination or further assessment is required

The requested statement cannot be supported by the medical record

There is a potential conflict with professional, legal or ethical responsibilities
Payment is for the professional time required to assess and process the request. It does not guarantee that the clinician will provide the outcome or opinion requested.
Requesting a non-NHS service
To request a letter, form, report or private medical examination, please contact the reception team and provide:

Your full name and date of birth

Details of the document or service required

The organisation requesting the information

Any relevant deadline

A copy of the form or written requirements
The practice will review the request and confirm the applicable fee, payment arrangements and expected completion time.
